Looking for a dynamic, detail-oriented, and experienced Contract-to-Hire Case Manager to join our team. The ideal candidate should have a strong background in client intake processes, managing programs, and handling confidential client files. This is a contract position that will require you to work closely with our clients, providing them with exceptional service and support. You will be responsible for managing a caseload of clients, coordinating various services, and ensuring that all client information is kept confidential and secure.
Responsibilities:
1. Conduct comprehensive client assessments to identify needs, goals, and potential barriers.
2. Develop, implement, and review individualized case management plans.
3. Coordinate and provide access to a range of services including healthcare, social services, and legal aid.
4. Maintain accurate and up-to-date client files, ensuring all information is kept confidential.
5. Monitor and track client progress towards their goals, adjusting case plans as necessary.
6. Advocate for clients, liaising with other professionals and service providers as needed.
7. Participate in team meetings, providing updates and feedback on client cases.
8. Ensure all case management activities comply with professional standards and organizational policies.
9. Provide crisis intervention as needed, and refer clients to appropriate resources.
10. Provide regular reports on case management activities, outcomes, and client satisfaction.
Qualifications:
1. Bachelor's degree in social work, psychology, or related field. A Master's degree is preferred.
2. A minimum of 2 years' experience in case management, preferably in a non-profit or social services setting.
3. In-depth knowledge of the client intake process, program management, and maintaining confidential client files.
4. Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and stakeholders.
5. Strong organizational and multitasking skills, with the ability to manage a large caseload effectively.
6. Proficiency in using case management software and other computer applications.
7. Ability to work independently, make decisions, and solve problems effectively.
8. Knowledge of community resources and services for vulnerable populations.
9. Ability to handle sensitive information with the highest degree of integrity and confidentiality.
10. Certification in case management is a plus.
