What You Will Be Doing:
- Manage a caseload of 12-15 hospice patients
- Schedule and coordinate patient visits, grouping them together for efficient care delivery
- Work directly with leadership to address and resolve challenges
- Provide compassionate end-of-life care and support to patients and their families
