Short Description
We are seeking compassionate individuals to join our team as community healthcare providers. In this role, you will work closely with medical professionals, primary care teams, health coaches, and other agencies to improve patient care and outcomes. Your support will help patients and their families develop the skills and knowledge needed to manage their health conditions effectively. You will work in various settings, including hospitals, primary care and specialty practices, patient homes, and community centers.
Job Duties:
- Identify and address the unique needs of patients by connecting them to appropriate support services (emotional, financial, administrative, or cultural).
- Build positive relationships with patients, providers, case managers, and community-based organizations.
- Assist patients in developing health management plans and goals.
- Follow up with patients and providers on their health management and care plans.
- Coach patients in effectively managing their chronic health conditions and self-care.
- Help patients understand their care plans and instructions.
- Collaborate and work effectively within a team.
- Establish supportive relationships with participants and provide feedback.
- Assist clients in accessing resources and health-related services.
- Stay updated on community resources and expand your knowledge.
- Facilitate communication and coordinate services between providers.
- Motivate patients to actively participate in their health management.
- Work effectively with people from diverse backgrounds, reducing cultural and socio-economic barriers.
Experience Required
- Preferred experience working in a community-based organization/environment.
- Preferred home-visiting experience.
- Experience with community collaboration.
Education Requirements
- High School Diploma or equivalent required, some college courses preferred.
