POSITION SUMMARY:

Project R.I.T.E is a SAMHSA funded program in support of the LGBTQAI community in navigating health care and social supports including HIV/HCV Prevention and Care. Project R.I.T.E focuses on aiding LGBTQIA communities color with increased risk for HIV/HCV, living with HIV or have a co-occurring mental health disorder.                   

The Project R.I.T.E Outreach Coordinator will work closely with LGBTQIA communities, providing HIV education and health promotion services. They will play an integral role on the team and is responsible for building and maintaining community partnerships, coordinating HIV/HCV testing and PrEP and PEP referrals with community partners, and planning and leading outreach events and activities (virtual and in-person).

Project R.I.T.E Outreach Coordinator will work collaboratively with other team members (Case Manager, RITE Counselors, RITE Support Specialist) and across programs within the Wellness and Social Services Department and with other primary care services at J.P.A.F.H.C.                                                                         

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Identify and develop new partnerships and collaborations with community stakeholders through Memos of Understanding (MOUs) to coordinate and conduct HIV/HCV testing and PrEP and PEP referrals.
  2. Sustain strategic partnerships and collaborations with existing community partners.
  3. Ensure MOUs and Linkage Agreements are up to date with community partners.
  4. Lead all outreach events and coordinate with RITE Counselors for testing and tabling events.
  5. Plan, implement and track all face-to-face outreach activities. These include street and community-based health fairs at community celebrations, colleges, universities, community centers, etc. HIV awareness days, Gay Pride celebrations, Transgender Day of Remembrance, and other social and cultural events honoring and celebrating LGBTQ communities of color. 
  6. Plan, implement, and track all online (virtual) outreach activities, focusing on digital and social media outreach strategies for Facebook, Instagram, Snapchat, TikTok, Twitter, etc.
  7. Market programs and services to engage priority populations through the development and distribution of innovative content.
  8. Prepare and submit outreach report as required by the Program Manager.
  9. Attend and occasionally present at monthly network meetings.
  10. Assure completion of data entry in (SPARS, CareWare, and EMR).
  11. Identify best practices for venue based as well as social media outreach.
  12. Work closely with RITE Counselors, Case Managers, and other departmental and organizational programs to refer and link clients/patients to Project RITE services.
  13. Assist the Director of Programs and Training on special projects.

MINIMUM QUALIFICATIONS:

  1. Bachelor’s Degree in Human Services, Marketing, Public Health, or related field preferred, but equivalent community work experience and/or other certifications will be considered for exceptional candidates. 
  2. Members of the LGBTQIA community are strongly encouraged to apply.
  3. At least two years of outreach experience preferred. 
  4. Experience in HIV prevention and treatment in healthcare or social service settings preferred. 
  5. Computer literacy with proficiency in MS Word, Excel, PowerPoint, Social Listening, and Data Analytics.
  6. Experience with Electronic Medical records preferred. 
  7. Demonstrate organizational, interpersonal, oral, and written communication skills.
  8. Must be able to prioritize and carry out multiple and diverse assignments and quickly pivot when needed.
  9. Must be able to work independently and collaboratively in and across teams and occasionally from a remote environment.
  10. Must have cultural competency and comfort working with LGBTQ populations. 
  11. Bi-lingual (English/Spanish) preferred.
  12. NYS Drivers’ License with vehicle preferred.
