Title: QA LVN HOSPICE NURSE
The Quality Assurance Nurse serves as a critical link in the identification and resolution of issues affecting hospice. Integrates the quality management/performance improvement program with clinical and business goals and objectives. Participates in gathering, trending and reporting data related to the oversight, planning and coordination of Quality management activities, including patient care reviews, compliance audits, concurrent and retrospective clinical record reviews for the interdisciplinary team. Trends and analyzes utilization, outcome and quality data.
Job Summary:
Primary function is to oversee and monitor the quality improvement program for the agency, perform duties and responsibilities related to QA.
Reporting: Reports to the Director of Nursing
Job Qualifications:
· Education - A graduate of an approved school of professional nursing who is licensed in CA. Candidates MUST be a graduate of an accredited school of vocational nursing (LVN). Although, RN’s will also be considered.
· Experience - Minimum of one year nursing experience ihospice and knowledge of QA Standards
· Skills - Ability to establish and maintain effective working relationships and communication with staff, management, physician and the governing body.

Works in an office environment; Ability to work a flexible schedule;
Physical and Mental Effort:
Prolonged sitting is required. Requires ability to handle stressful situations in a calm and courteous manner at all times. Requires working under some stressful conditions to meet deadlines and agency needs.
Essential Functions / Responsibilities:
· Complete clinician visit note review / QA;
· Assisting with the overall development and implementation of the QAPI program; identification of goals and related patient outcomes; assisting in the identification of goals and related patient outcomes; participating as a member of the QAPI Committee; assisting in the development of the quarterly QAPI Reports and Annual QAPI Summary Report.
· Investigation and actions on matters related to standards in areas of misconduct/incidents/adverse events, including coordination of internal investigations and any resulting corrective action with all departments and health care professionals under the home health agency's control.
· Reporting to the home health agency’s Director of Nursing regarding the progress of development, implementation, and establishment of measurable patient outcomes, agency's efficiency of services and quality of care. to improve the home health agency' s efficiency and quality of services and ensure compliance with standard of care and applicable regulations.
· Demonstrates an in-depth knowledge of, and ensures compliance with, all local, state and federal laws relating to all of the operations of the agency.
· Order and maintain an inventory of medical supplies for the agency.
· Carries out other duties as assigned by the Director of Nursing, Administrator, and the Board of Directors.
- The QA Nurse will audit all charts submitted by our clinicians to assure all are in compliance with State and Federal regulations. This position is primarily office based. In this role you will review all clinical documentation and checking for completion and accuracy. You will be working with the Clinical Director, clinical and office team to support workflow functions that impact chart review.
- Ensures compliance with all state, federal, and referral/intake regulatory requirements.
- Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Review all assessment documents for accuracy, consistency and supporting documentation
- Review clinical charting for conformance/compliance to Medicare and Agency standards
- Discuss any noted documentation discrepancies with Supervisors and clinical staff as appropriate
- Participate in quality assurance and improvement activities for the company.
- Participates in quality assessment performance improvement teams and activities.
- Other duties as assigned.

Pay: $40.00 - $60.00 per hr
Minimum Requirements:
- Minimum LPN/LVN licensure
- Minimum (2) years of experience as a nurse, hospice nursing strongly preferred
- Preferred: 1 year minimum QA experience
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possesses and maintains current CPR certification REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
- Knowledge of federal, state and accreditation standards applicable to hospice required
- Knowledge of governmental and other regulatory standards, requirements, and guidelines related to quality improvement
- Strong working knowledge of ongoing monitoring techniques (including criteria development and statistical analysis); care delivery in community; total quality management principles, tools, and techniques
- Ability to effectively communicate, negotiate and lead
