What are the responsibilities and job description for the Coordinator, Population Health (Non-Clinical) position at Kaiser Permanente?
The Population Health Program offers an innovative, evidence-based health care program that improves health outcomes for low-income and/or high-risk patients. The Population Health Coordinator identifies high risk patients, coordinates program activities and assists with analysis of data. Plans, organizes, and distributes resources to meet patient care needs. The Population Health Coordinator will play a critical role in achieving the triple aim of enhanced quality, reduced cost and improved health outcomes for the high-risk patient population.
The Coordinator supports the social, economic and behavior needs of the vulnerable populations within Kaiser Permanente and for the communities we serve. The Coordinator works on an inter-disciplinary care team support for members. The Coordinators are highly trained communicators and skilled in motivational interviewing and patient centered care. Under the overall general guidance of a Director and/or Manager, responsible for coordination of members for our community health navigator, researching data, performing analyses, presenting results, and making recommendations. Acts as program/policy specialist to assist department and/or cross-divisional groups with assignments/initiatives as required.
