Job Summary:
The Clinical Care Coordination Specialist will provide seamless transition from diagnosis to treatment, ensuring that internal and external care are integrated, progress is monitored, and appropriate, and evidence-based treatment is delivered.
Essential Responsibilities:
Coordinates the care of a large caseload of patients between KP providers and external contracted providers. Acts as liaison to outside providers.
Reviews medical and mental health history to accurately report to outside providers.
Reviews treatment plans and conducts chart reviews, to monitor progress, utilizing evidence-based guidelines and other established criteria. Reports all potential risk situations per guidelines.
Makes recommendations for referrals per guidelines.
Develops and maintains relationships and effective communication between external providers and all levels of medical center providers to ensure effective care coordination.
Participates in multi-disciplinary clinical consultation meetings as needed for monitoring progress and coordination of care.
Provides psycho-education and community resources to members as needed.
Develops and maintains relationships and effective communication between patient/caregiver, external providers and all levels of medical center providers to ensure effective care coordination.
Supports clinical team in updating and disseminating treatment plans.
Basic Qualifications:
Experience
Minimum of two (2) years of behavioral health case management or mental health clinical experience in a large health care organization or community based setting.
Education
Masters degree (Social Work or Psychology), or PhD (Psychology) required.
License, Certification, Registration
Psychologist License (California) OR Licensed Marriage and Family Therapist (California) OR Licensed Clinical Social Worker (California)
National Provider Identifier required at hire
Additional Requirements:
Experience following evidence-based treatment guidelines.
Responsibility for a large caseload, for an extended period of time.
Knowledge of local, state, and federal regulations.
Must be able to work in a Labor Partnership Environment.
