What are the responsibilities and job description for the Quality Auditor - RN or LPN / LVN position at Kare-In-Home?
The Quality Auditor (QA) participates in the organization-wide ongoing internal auditing process to assess for compliance in adherence with current federal, state, and internal guidelines, and regulations. The QA collaborates directly with clinicians and branch leaders to improve clinical documentation, accuracy of OASIS comprehensive assessments. Collects and analyzes real-time quality data and reports to the Administrator and Branch Director to keep them abreast of findings. Provides a quarterly summary audit findings and uses internal tools to trend results.
Responsibilities:- Performs routine internal chart audits, as assigned, reviewing both electronic and paper visits inclusive of all Disciplines.
- Utilizes RAT STAT tool to identify record review selection.
- Clinical Audit focus to include, but is not limited to, review of documentation to support: homebound status, individualized Plan of Care with measurable outcomes and goals, services are provided in accordance with the Plan of Care, appropriate physician orders, accuracy of the Medication Profile, wound care documentation, infection prevention, etc.
- Assist Administrator/BD to establish appropriate action items and improvement plans.
- Send Rapid Responses directly to the clinician identified on audit related to wounds and copy BD/Admin.
- Coordinate education needs to the Clinical Education Managers and QOS team to coordinate follow up ZOOM meeting to clinicians identified on the audit for wound care, medication profile, alerting the physicians to changes in patient status, and coordination of care. Education Zoom meetings shall be no longer than 30 minutes in duration and targeted to action items. Documentation of attendance is provided to the branch to include as part of the performance improvement plan.
- Assign appropriate remedial education modules and track completion within the LMS system.
- All audit summaries and performance improvement plans (PIPs) will be uploaded to the Branch Folder- Quality Folder.
- Enters audit findings and result in the Quality Audit tool for trending purposes.
- Sends an immediate alert via email to the ADM & BD for immediate clinical concerns, patient care issues or safety issues identified during the record review for a rapid response.
- Follows up with the ADM/BD to ensure there is a 24-hour turnaround on all rapid response alerts.
- Collaborates with the Clinical Team to support additional education and audit needs.
- Participates/keeps abreast of continuous learning, education, and regulatory changes in OASIS, HIS, Quality and POC development, as necessary.
- Follows agency policies and procedures. Maintains confidentiality of patients and agency information per HIPAA requirements.
- Enhances professional growth and development through participation in educational programs, staff in-service and workshops.
- Graduate of a School of Registered Nursing (RN) or School of Vocational/Practical Nursing (LVN, LPN),
- Nursing compact licensure required if allowed in state of residence. If not, must be willing to obtain licensure
- OASIS Certification preferred, but not required.
- Current Driver’s License in state of residence.
Experience:
- Minimum of two (2) years’ experience in home health required.
- Demonstrated experience in teaching within the healthcare setting, preferred.
Skills:
- Demonstrates a current working knowledge of Medicare guidelines for home health coverage and
- Knowledge of the “Conditions of Participation” for Home Health including but not limited to: Physician Face
- Knowledge of the documentation required to support homebound status for patients, patients having a
intermediate skilled nursing, physical therapy, speech therapy, or the continuation of occupational therapy.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental/Working Conditions: Performs duties in an office environment during Agency operating
hours. Works in a routine office environment; noise level may be moderately high; must have the ability to work
a flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc. The ability to work a
flexible schedule; ability to travel locally on occasion; some exposure to unpleasant weather.
