The Elderly Services Coordinator is responsible to coordinate, implement and monitor development plans and support services for Home Delivered and Congregate clients to receive nutritious meals and other services so that they are able to remain living independently. The Coordinator will monitor the client's participation and progress, assisting them in overcoming barriers in health, nutrition and in other areas as needed. Duties include outreach, recruitment, intake and assessment of clients needs though scheduled home visits for Home Delivered Meals clients and site visits for Congregate Meals clients. Complete intake applications, determine eligibility and assist in enrolling eligible clients into the program. Identify immediate and long term needs of the participants and develop individual service plans. Schedule and provide nutrition education sessions and activities. Refer participants to appropriate KEO programs and/or other agencies for assistance and follow up as needed. Other duties include assisting with packing and meal delivery, Emergency Food distribution and Persons In Need Program. Prepare, maintain, and file monthly reports, rosters, attendance sheets, and route manuals. Qualified applicants will possess a BA degree and one year of experience or any comparable combination of education and/or experience which would provide the knowledge, skills, and abilities to perform the essential duties of this position. Degrees relating to social services preferred. Must have access to a vehicle, possess valid driver's license, current auto insurance, clean traffic abstract and background clearance.
