Supported Living House Manager
- Maintains a routine minimum of 35 hours of on-shift work hours providing direct support for persons supported in their homes and communities
- Conducts and coordinates in-service training for Direct Support Professionals, including individual specific training needs, as well as any other training needed
- Supervises, mentors, and delegates tasks to Direct Support Professionals and reports employee issues and events to supervisor as instructed
- Creates weekly schedules for staff and completes and turns in time sheet audits
- Assists people with maintaining their personal funds in accordance with their ISP, including maintaining receipts and ledgers and safeguarding the person's funds
- Performs grocery and supply shopping, including ensuring sufficient food and supplies are always present in peoples' homes
- Coordinates and provides back-up care in case of a staff emergency or unexpected event
- Remains on-call to address emerging or urgent needs, provide backup care, and coordinate events, activities, and appointments
- Assures time off for the direct support staff, as needed, with a focus on reducing and avoiding unnecessary overtime
- Assures the person's safety through observation and timely reporting for neglect, abuse, and/or exploitation and reporting in accordance with all policies, rules, regulations, and laws
- Checks and reviews daily documentation for errors and issues on an ongoing basis, at least one time weekly
- Attends medical appointments and communicates medical appointment information and documentation to Medical Director in a timely manner
- Attends ISP and COS meetings, advocates for the person supported, and participates in the person’s Circle of Support
- Relays pertinent/required information and documentation to Agency Administration upon request and as instructed
- Checks MARs and medication documentation on an ongoing basis throughout each month
- Administers medications in accordance with DIDD requirements and schedules medication passes for times that a certified employee is not on shift
- Provides transportation to and from appointments, community activities, and other events of the person's choice
- Actively engages in assisting the person in completing their personal goals/outcomes and continuously monitoring for progress and opportunities

Monday-Friday
Pay Range-
$16.00-$17.00/hr. depending on experience.
