What are the responsibilities and job description for the Service Coordinator position at KFI?
The Service Coordinator is responsible for ensuring that the needs and dreams of people supported are accurately reflected in KFI generated documentation, including but not limited to, Service Implementation Plans and daily documentation. The Service Coordinator also promotes the principles of SRV (Social Role Valorization), and acts as the primary liaison with DHHS and Community Case Managers. This is accomplished through close collaboration with the person, their KFI Support Coordinator, family members, Community Case Managers, KFI staff, members of the community, and state agency personnel.
Responsibilities:
1. With persons supported, arrange and facilitate pre-planning and discovery meetings; develop written Service Implementation plans and enter information into state systems in a timely manner, ensuring compliance with annual plan requirements
2. Maintain a schedule of regular in person visits with all people on the caseload to develop and maintain an understanding of each person’s support needs.
3. Communicate with Community Case Managers and other external stakeholders regarding planning meetings and other service related requirements as needed throughout the year.
4. Collaborate with the Team Support Coordinator to ensure that each person's needs are accurately reflected in all documentation.
5. Create daily documentation (Individual Support Plan) programs that accurately reflect the goals and desires of the person supported and update annually and as needed. Assist with Monitoring of Therap documentation on a regular basis.
6. Maintain working knowledge of State and Federal regulations pertaining to support provided. Ensure compliance with Home and Community Based Supports (HCBS) regulations by completing forms with each person on caseload and updating annually or as required.
7. Participate in management meetings, discussions and activities relating to the development, improvement, and expansion of KFI services.
8. Any other duties needed to fulfill our mission to empower and support people with disabilities in their efforts to live full lives as integral, respected members of the community.
