Job Summary
The Case Manager, Hospital, is responsible for the coordination and completion of all tasks, documentation and communication relevant to the provision of case management support service beginning with admission and ending after the client's first post-discharge appointment.
Job Duties/ Essential Functions
- Review Initial Assessments immediately after admission to insure accuracy. Document any and all discrepancies in the applicable spreadsheet.
- Notify all current providers of admission within twenty-four (24) business hours of admission. Respond to all customer requests within forty-eight (48) business hours.
- Notify all involved county agencies, including schools if there is a Release of Information (ROI), of admission within twenty-four (24) business hours of admission and provide clinician contact information of same.
- Work in collaboration with the clinician to insure accurate aftercare and/or to insure that all collateral contacts are made.
- Document all necessary information in the applicable clinical system.
- Fully perform restraints as necessary when ordered by a licensed professional using the current approved KidsPeace restraint methodology on youth who may weigh in excess of 250 pounds and who are verbally or physically threatening or otherwise presenting a threat to themselves or others.
- Assume other tasks and duties as assigned by leadership.
Qualifications (Education, Training and Experiences)
- High school diploma (or equivalent) required. Bachelors degree in appropriate discipline preferred.
- Prior experience working with children preferred.
- Must be at least 21 years of age.
- Valid driver's license, SCM certification, CPR/First Aid certification required.
- Basic computer skills required.
