We are searching for a Licensed Practical Nurse – someone who works well in a fast-paced setting. In this position, you will work collaboratively with the IDT members and Patient Care Managers (PCM) to provide coordination of patient care.

Job Duties & Responsibilities
- Perform chart audits under the direction of the Clinical Quality Performance Coordinator in a timely manner to ensure efficient and effective patient care
- Coordinate daily electronic scheduling and completion of audits in the HAA scheduler’s absence
- Be a link between the PCM, nursing staff, billing, intake and pt/family to ensure quality patient care and perform phone surveys to ensure satisfaction of hospice services
- Collaborate with primary nursing staff, PCM and other IDT members regarding continuous care and will make LPN assignments
- Demonstrate good communication skills and clinical judgment with patients/family and staff
- Responsible for accurately maintaining the master schedule for all clinical staff to adequately meet the needs of patient census as outlined in scheduling guidelines
- Enter orders into the EMR within the framework of licensure requirements
- Perform HIS reporting as mandated by Medicare for quality reporting in alignment with the Affordable Care Act of 2010
- Verify benefit payor source prior to dispatching continuous care staff and obtain precertification from payor source if needed and enter into EMR
- Keep accurate record of DME signed out of office and provide monthly report of this usage to billing department
Skills & Requirements
- Georgia LPN Licensure Required
- Current BLS Certification Required
- CHPLN Preferred
- Two Years of Healthcare Experience Required
