Panel Coordinator
Location: Montpelier, VT
Travel Pay Rate: $55/Hr. on 1099
Local Pay Rate: $35/hr on 1099
The Team-Based Care Housing Panel Coordinator works with AHS Field Services Program to help coordinate services and collect data from partners working with program participants in Team Base Care program to track progress and outcomes. The TBC Housing Panel Coordinator will also contact program participants to check in on their status and current needs. In some cases, service connections will be required. Data collection and reporting is required. The position also works as part of an interdisciplinary team of providers to track participation and progress of participants. The position also supports Field Directors and VCCI staff in implementation of team-based care ecosystem in the community and works with the team to develop strategies to document and track outcomes at individual and systems levels.
DUTIES
Develops a keen understanding of team-based care practice requirements for optimal, coordinated population health and works as an effective team member of the program.
Manages data collection using CommCare, prepares reports on findings, and provides interpretations and recommendations to each practice.
Contributes to quality improvement and care redesign of population health efforts.
Reviews patient’s pertinent screening results and care team data and consults with care team to ensure services are provided and outcomes updated in CommCare.
Manage reports and provides the members of health care teams in community with the data and/or eligibility changes required to meet the health needs.
Supports Field Director and team-based providers to develop creative processes to proactively manage target populations.
Provides proactive outreach including calls and/or meetings with identified participants to assist in organizing or planning next appointment (i.e. no shows).
Provides data management, coordination, and patient outreach as needed for specific target patient populations.
Contributes to a positive experience for patients and families through courteous telephone interactions and interview activities, accurate and expeditious routing, as well as referral to appropriate clinical staff when necessary.
Respond to inquiries from the practice regarding outcomes of outreach calls to patients.
Synthesizes, formats data, and generate ad-hoc reports. Presents data to each of their assigned share care plan providers.
Assists in process mapping and development of workflows for team-based care ecosystems within region.
Participate effectively as a team member, attending practice staff/team meetings and working closely with all members of the health care team to improve quality of care, while supporting Agency of Human Services goals and objectives.
Employees will perform their jobs in a safe manner as defined in any and all applicable AHS policies specific to the job including but not limited to policies addressing: universal precautions, use of personal protective equipment, safe lifting/ergonomics, handling of hazardous materials.
Adhere to relevant Human Resources employee policies including HIPAA and 43 CFR Part 2 polices.
Notifies direct supervisor of infractions of policy, procedure, laws and regulations as they are identified. Notifies supervisor’s manager of same issue if supervisor does not correct or resolve issue within a reasonable time frame. Notifies CVMC Corporate Compliance Officer of any issues that continue to be unresolved.

BASIC KNOWLEDGE
Proficient in all Microsoft Office applications, electronic health record (EHR), and process improvement experience (PDSA) required. The understanding and practical use of databases and to effectively communicate through the use of reports, graphs, and supporting narrative is a must. The incumbent must possess strong and effective organizational skills; effective and tactful written, verbal, and nonverbal communication skills; the ability to recognize opportunities for improvement through patient, clinician or staff interactions; professionalism, sensitivity, empathy and respect; and must be a proactive/forward looking individual. Proficiency in Motivational Interviewing and interacting with people who have complex health and social needs. Must also be able to work with providers in all domains of care to convene and facilitate meetings.
MINIMUM REQUIREMENTS
BA in Human Services field OR Community Health worker with experience in human services.
EXPERIENCE
Minimum of three years of human services experience required. Prior use of electronic health records and other health care information systems desirable.
