Koinonia Homes for Teens (HFT) is seeking a qualified individual to serve as the Case Manager for its Crisis Resolution Center in Loomis, California. The program serves youth and families from Placer County who are at risk of placement in the child welfare system. The Case Manager provides counseling, family meetings and referrals to community resources for the youth and families served by the program. The Case Manager also supervises several direct care staff who support the program.
The Case Manager opportunity includes:

The Crisis Resolution Center in Loomis, CA serves youth in a residential setting who are self-referred or referred by parents, law enforcement, school or Placer County. The youth are referred during times of personal or family crisis and are in care for up to 30 days with the goal of reunification with their family.
The successful candidate will hold registration as an ACSW, APCC or AMFT, have experience working with youth with complex trauma, team well with others, have excellent communication and organizational skills, strive for ongoing professional development, and have a high degree of integrity and ethics.
Duties and responsibilities include:
- Maintains and provides positive public relations with youth families and community
- Works cooperatively with the treatment team and Placer County team on client treatment plans
- Collaborates to create a treatment plan and goals for each client
- Provides counseling to youth and their families
- Provide direct training and supervision of all direct care staff
- Provide a full range of interventions including behavioral management, interpersonal effectiveness skills, interventions with schools and parents, family support and training, community transition, and self-management
- Maintain case files for each assigned youth
- Remains on-call and responds to staff, client and facility emergencies as needed.

Must hold and maintain a group home administrator’s certificate issued by Community Care Licensing. Must have a combination of education and experience to meet the qualifications of a group home administrator as specified in ILS 87064. Generally, the qualifications are a master’s degree and one year of experience, a bachelor’s degree and one year of experience, or two years of college courses plus two years of experience.
