CCSS/BMS Team lead

Job Description:

The team lead coordinates and provides services and resources to individuals/families necessary to promote recovery, rehabilitation and resiliency. Resources provided seek to improve patient functioning in the following areas: parenting skills; independent living; learning; socializing and recreation. The Team Lead provides programmatic support and oversight for La Clinica de Familia’s CCSS, BMS and Supportive Housing programs. Duties include, but are not limited to: staff assignments and supervision, tracking team documentation and productivity compliance, and conducting Continuing Quality Improvement (CQI) meetings. The Team Lead is responsible for ensuring the effective provision of services and for the administrative supervision of the team and tracking of required data metrics and for presenting data to leadership. The Team Lead also encourages each team member to fulfill their goals and to function at their highest potential following a core set of principles and values. The Team Lead instructs and teaches team members by providing materials and knowledge on various aspects of treatment and topics of interest. The Team Lead performs these duties under the supervision of the Clinical Supervisor/Director.

Job Requirements:

Minimum staff qualifications for the Team Lead Must hold a Bachelor’s Degree in a human services field from an accredited university; and, • Have four (4) years relevant experience in the delivery of Comprehensive Community Support Specialist (CCSS) with the target population; and, • Have one (1) year demonstrated supervisory experience.
