What are the responsibilities and job description for the Pharmacist - FQHC Sites position at Lake County?

Summary
Responsible for the provision of virtual and in person pharmaceutical consultation within the scope of practice, in accordance with established standards of care. Works with providers and primary care leadership to achieve clinical and practice goals set by LCHD/CHC. This position is directly supervised by to the medical director.
Scheduled Hours: As needed
- Directly provides pharmaceutical consultation and treatment recommendations for patients to providers
- Provides education to providers as needed or requested
- Meets with patients concerning their medications, when needed.
- Comfort with working with various providers at various hours on an as needed basis
- Exhibits a patient focus and comfort with carrying for varying patient populations
- Participates and presents at clinical conferences and provides medical expertise as appropriate
- Reports all unusual occurrences that may involve the safety and well-being of the clients, patients and/or co-workers, directly to the Practice Manager and/or Safety Coordinator.
- Performs duties in a prioritized, organized and orderly manner to maximize clinic efficiency and productivity.
- Requires completion of a pharmacy training program.
- Must be at licensed as a pharmacist
- Academic appointment at a Medical School preferred.
- Ability to work on weekends and evenings if necessary.
- Demonstrated experience as a strategist. Can set priorities, establish clear (and high) expectations and execute identified strategies.
- Ability to maintain and keep records.
- Ability to work part time, with different providers and patients focus on an on-call basis
- Ability to establish and enhance working relationships with community, governmental and professional health personnel, federal and state officials, and the general public.
- Working knowledge of PC operations and software (Word processing and spreadsheet), and good keyboard skills.
