Position Summary:

The Medical Records Coordinator/Clerk will provide for the management and organization of the patient medical records; both electronic and hard copy in compliance with all regulatory and confidentiality requirements.

The Receptionist is responsible for greeting, instructing and scheduling patients. Serves as liaison between patients and the clinic staff. Performs and follows through with clerical and secretarial activities per office guidelines inherent to the operations and needs of the department.


  • Essential Duties and Responsibilities include the following:

The Medical Records clerk is responsible for:

1. Scan and attach appropriate documents to patient electronic records with accuracy

2. Maintains orderly files.

3. Checks continually for misfiled information

4. Copy and/or printing of records for transfer

5. Maintain tracking of records transfers and copying

6. Copy records for patient needs

7. Obtain and document all pertinent information in patient's charts.

8. Maintain patient safety and confidentiality.

9. Assist with training and orientation of new personnel.

10. Assure the readiness of the reception are for each working day; open the office at the designated time and have all front desk activities fully operational at the start of business hours.

11. Assist in obtaining all necessary records/test results prior to patient appointments.

12. Responsible for updating patient demographic information on computer system.

13. May be responsible for verifying insurance coverage/eligibility

14. Collects patient's co-payments and payments.

15. Obtains and scans current copy of patient's insurance ID cards.

16. Maintains and submits batch information-daily report and cash

17. May be assigned to complete daily productivity reports.

18. Verify that all visits to a specialist / for specialty services (if applicable) have prior authorization per office guidelines.

19. Keep work area, lobby & conference rooms neat and organized.

20. Answer all incoming calls in a pleasant and professional manner, route to the appropriate person and/or take message.

21. Greet all walk-in traffic in a pleasant and professional manner, determine whom they need to see and call that person to the front to meet their visitor.

22. Smile and make eye contact when greeting patients and other customers.

23. Demonstrate care, concern and courtesy when interacting with all internal or external customers.

24. Display warm and pleasant personality, emphasizing tact, empathy and patience.

25. Display professionalism, good judgment, maturity and fairness when interacting with others.

26. Ability to handle a multitude of assignments, meeting all given deadlines

27. Responsible for incoming/outgoing schedule of personnel and visitors log.

28. Responsible for incoming/outgoing packages (UPS, FedEx) and package log.

29. Maintain conference room calendars/schedules.

30. Ability to stay busy during quiet times in front, e.g. sort paperwork, collate mail, encounter data.

31. Adhere to company policy regarding unauthorized use of Internet for personal gains during work hours.

32. Any other projects as requested by administration.

33. Know location of Policy and Procedure and Communication books and be familiar with contents.

34. Assist Supervisor with special duties/projects.

35. Universal precautions utilized

36. All other duties as directed by management.


  • Education and/or Experience:

1. High School graduate equivalent.

2. One year experience in a physician office or clinic with emphasis on front office/medical records

3. Good written and verbal communication skills.

4. Above average computer skills, EMR experience preferred.

5. Knowledge of Windows-based computer skills preferred and Microsoft office programs.

6. Well organized.

7. Ability to work well under pressure.
