What are the responsibilities and job description for the Bilingual Licensed Practical Nurse position at LCH Health and Community Services?
The primary function of the Licensed Practical Nurse is to provide chronic care management to high-risk patients, perform triage (phone and in-person), and assist in the care primary care patients, Women’s Health (WH) and Pediatric patients.

Essential Duties and Responsibilities
- Team based care is the provision of patient-centered comprehensive healthcare by a core interdisciplinary team of healthcare professionals, including the Provider, Registered or Licensed Practical Nurse, Medical Assistants, Patient Service Representatives, and ancillary services. Team based care for the Licensed Practical Nurse includes motivational interviewing, healthcare coaching, chronic disease and population health management, and management and supervision of unlicensed staff.
Minimum Qualifications
- Bilingual proficiency in English and Spanish required
- Graduate of an accredited school of Licensed Practical Nursing
- Hold current, or eligible for, LPN license in the Commonwealth of PA
- Holds current CPR certification
