Responsibilities:
- Manage a small caseload of 15-18 clients who receive in-home personal care, coordinating services and ensuring care objectives are met by Direct Support Workers (DSWs).
- Meet with potential new clients, conduct intake visits, and assist the client/family through the onboarding and staffing process with Leading Home Care.
- Work closely with the Marketing department to promote Leading Home Care in the Eunice area, making connections in the community and building relationships with external stakeholders.
- Serve as point of contact for the Eunice location, working under the direction of leadership in Opelousas in Lafayette, and travelling to the Opelousas office as needed for training, meetings, and exchange of information.
- Draft care plans, complete check-in calls and visits, review case progress, and communicate frequently with clients, families, and staff regarding questions or issues with care services.
- Supervise, train, and direct DSWs in regards to all aspects of client care, ensuring that person-centered care plans are carried out and clients' health and well-being are maintained.
- Travel to clients' homes for meetings and visits quarterly, annually, and as needed, or as required by program guidelines.
- Follow proper documentation and reporting procedures for incidents, grievances, medical changes, reports of abuse or neglect, and any other client communications or issues.
- Comply with all program guidelines and meet requirements established by LDH, Medicaid, and LHCL.
- Participate in after hours on-call rotation and serve as backup staff working in-home with clients as needed.

Requirements:
- BA/BS degree in human services, social services, family studies, healthcare administration, or other related field
- Proven working experience in case management, administration, or healthcare - 2 years preferred
- Satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driving, and other checks
- Basic computer skills including Microsoft Office, Word, and Excel
- Able to operate office equipment
- Basic addition/subtraction and general analytical skills
- Excellent communication, organization, and time management skills
- Valid driver's license, auto insurance, and reliable transportation
Office hours are 8:30 a.m. - 4:30 p.m. Monday through Thursday and 8:30 a.m. - 4:00 p.m. Friday.
