Learning Services, A Collage Rehabilitation Partner is looking for a Full Time Case Manager in Roswell, GA. Learning Services is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

The Case Manager is responsible for:
- Ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up
- Coordinates schedules, therapies and other appointments/services
- Responsible for regular contact with families and funders
- Report writing, clinical documentation and facilitating funding issues as needed.
- Acts as a client advocate
- A bachelor’s degree along with case management and health care industry experience is required. A master’s degree and CRC or CCM designation is preferred
- Must have a valid driver's license and maintain responsible driving behaviors
- Excellent planning, organizational, writing and time management skills are required
- The ability to communicate with families, professionals and co-workers is a must
- Strong clinical skills as shown by the ability to manage crisis situations, assess clients' needs and help formulate, prioritize and monitor treatment plans is necessary
- The ability to demonstrate positive client interaction skills is essential
