Summary:
Provides specific activities of daily living for clients such as, bathing, grooming, ambulating, transferring, shopping and transporting for varied time frames to include 1-2 hour visits or 4-12 hour shifts in a clients home, hospital, ALF or convalescent facility. Observes client condition, documents condition and reports changes according to the Service Provision Plan or Care Plan. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering weekends and team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our patient caseload and the needs of the organization
