Job SummaryThe North Team travel requirements will be: North Fort Myers and Cape Coral areas.Eligible for up to $10,000 in Bonuses!The registered nurse (RN) often makes the first visit to assess the patients health.

Summary:
Coordinates telemonitoring systems and processes for LMHH to improve patient outcomes and reduce readmissions to the hospital. Demonstrates exceptional triage skills and appropriate judgment/assessment skills to be able to make independent clinical decisions in monitoring patients' vital signs and conduct appropriate follow up as indicated. Has ability to deal with physicians, clinicians, and patients. Possesses computer skills in order to be proficient in all aspects of the monitoring station and runs various reports on a regular basis to support quality initiatives and ensure positive outcomes in patient care. Performs in home visits as needed or assigned by the supervisor. Scope of Duties: Professional nursing service is provided by a registered nurse and includes: Initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points Initiating the plan of care and revising as necessary Providing those services and/or treatments requiring substantial and specialized nursing skill Counseling the client and family in meeting nursing and related needs. Initiating appropriate preventive and rehabilitative nursing procedures Preparing clinical and progress notes Coordination of services Referral to other services as needed Informing the physician and other staff of changes in the client's needs Evaluating outcomes of care Supervising LPN/LVN's and paraprofessionals providing services Assigning home health aide to a specific patient Participating in in-service programs Planning for discharge from service Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health/Telehealth Team, and are expected in this role. Duties will include covering weekends and team member PTO.
