Social Worker - MSW Qualifications:
- Licensed in the state of Minnesota as an LGSW or LISW (Master's-Level Social Worker)
- At least one year of clinical Social Work experience
- Experience with Electronic Medical Records (EMR) preferred
Social Worker - MSW Responsibilities:
- Conduct Social Work home visits to assess patients' psychosocial status
- Provide Social Work services in the home, including short-term individual counseling, community resource planning, and crisis intervention
- Collaborate closely with all members of the health care team in developing the plan of treatment

Pay: Up to $52.00 per hour
