Responsibilities

• Fields incoming referrals and monitor all channels for referrals, including phone, e-mail, faxes and EMR, and respond in a timely manner.
• Communicates with all outside agencies to address any concerns with the referral process.
• Identifies and informs prospective patients and their families about the nature of ACHC’s services and set expectations for the delivery of those services; updates referral sources on status of referrals.
• Assesses new patient criteria for interested patients, including whether or not they are in a qualified area, have acceptable insurance and are either homebound or home limited.
• Initiates new patient protocol and schedules first appointment with provider for qualified new patients; enters all patient data into electronic medical records (EMR) and updates provider.
  
  
  

Education And Experience

Education Requirements

• High School Diploma or GED
• 2 years of Healthcare Industry related experience
