Responsibilities:
- Collaborate in developing, monitoring, and fine-tuning program plans.
- Source and streamline resources for individuals in need.
- Keep meticulous records of patient assessments, care plans, interventions, and successes.
- Facilitate and engage in interdisciplinary meetings with individuals, families, and service providers to ensure comprehensive care.
- 1 year of Case Manager or Social Work experience
- Bachelor’s degree or equivalent work experience REQUIRED
- Proven de-escalation and conflict resolution skills
- Fluent in Spanish
