The people Liberty serves require various levels of nursing, medical and psychiatric care – thus, nursing services are vital to our clinical program and every day care.
To support our residents, Liberty Healthcare seeks to hire a Licensed Vocational Nurse to provide onsite nursing assessment and direct care services within our homes. This role is very similar to that of a community-based home health nurse – with day to day responsibilities including:
- Assessing residents’ acute and chronic health status
- Administering medications and monitoring efficacy and side effects
- Triaging and referring to health care providers as necessary
- Educating and training residents and staff about symptoms, medications and general physical and mental health topics
- Functioning as a member or the program's multidisciplinary team and actively participating in team meetings
