Position Summary
Under the direct supervision of the Adult or Children’s Care Management Supervisor, the Children’s or Adult Health Home Care Manager will assist youth who are eligible for and in need of care coordination by identifying and addressing physical, behavioral and social health needs. The primary function is to establish an integrated care team of service providers and supports who will collaborate to ensure that youth or adults are accessing supports and services to reduce the risk of emergency room visits, hospitalizations and other out-of-home placements. Ultimately, this integrated model of care is designed to result in better care, better health outcomes and lower cost of care.
This position is full-time, Monday through Friday, 40 hours per week during regular business hours. The position operates in a hybrid work model with required travel to conduct home and community-based visits.
Job Responsibilities
- Work with youth and their families or adults to determine their immediate and ongoing eligibility for Health Home Care Management services and assist with completing the enrollment process.
- Conduct ongoing assessments to identify youth or adult needs, and develop comprehensive, family-driven care plans that establish clear goals to improve the quality of the youth’s functioning, health and wellness.
- Coordinate care and routinely collaborate with multidisciplinary team members including family members, schools, behavioral health providers, primary care physicians, managed care organizations, social supports and other specialty providers to ensure best quality of care is received.
- Refer youth or adults to health care providers, mental health and substance use providers, medications, housing, social services (I.e. food, benefits, transportation, etc.), and other community resources.
- Support and encourage youth or adults to increase knowledge about their mental and physical health conditions, engage in their care, improve adherence to treatments, and develop confidence in coordinating care independently.
- Conduct home and community-based visits to engage with youth, their families, or adults and other collateral supports.
- Manage an average caseload of 25-35 youth or adults with varying levels of need.
- Participate in the on-call rotation to handle after-hours emergency/crisis situations.
- Complete all required documentation within timelines in accordance with program standards.
Qualifications
- Minimum of a bachelor’s degree with 2 years of relevant experience in health or human services; or master’s degree with 1 year of relevant experience; or Registered Nurse with 2 years of relevant experience.
- Previous care coordination/case management experience working with youth and families or adults with Serious Mental Illness, developmental disabilities, substance use disorders, or chronic medical conditions preferred.
- Knowledge of community resources, Medicaid Managed Care and social service systems.
- Ability to understand and comfortably operate health information systems and electronic medical records.
- Demonstrated ability to think creatively and use a solution-focused approach to solving problems.
- Strong attention to detail and solid organization and time management skills.
- Must have a valid New York State driver’s license and access to reliable transportation.
- Must be willing to travel throughout assigned county to conduct home and community-based visits.
