Salary:$19.50-$22.50 Per Hour
MAJOR DUTIES: This position provides care coordination to agency patients with co-morbidities and other barriers. This position facilitates successful transition of care from inpatient settings to outpatient settings. Care Coordinators (CC) are designed to provide education and support to members with elevated risk and are elevated utilizers of health care services. This position provides clinical services to agency patients. In fulfilling these duties, the incumbent performs the following duties independently:
- The CC will spend time advocating, educating, and supporting patients and natural family supports to attain and improved self-management skills.
- CC will meet ongoing both in person and telephonically to support patients and the patient’s attainment of health care goals.
- Coordinating with the authorizing and health care or social service entities as necessary to maintain self-care and support goals.
- Provide support around medication and medication use. Assist patient with medication refills.
- Should facilitate successful transition of care from inpatient to outpatient settings. Supporting discharge instructions, coordinating with specialty and primary providers, reviewing care environments, ensuring medication is understood and used appropriately.
- Care Coordinator will document patient progress and recommendations in the electronic health care system. All in person and telephonic encounters will also be documented.
- Provide direct clinical support to patients, such as in group or individual settings.
- Work collaboratively among a team.
- Other duties as assigned.
KNOWLEDGE AND SKILLS REQUIRED BY THE POSITION
- Preferred a bachelor's degree in human sciences or related field. Highschool diploma required.
- Agency Affiliate credential required after hire date
- 1-2 years’ experience with coordinating care for patients.
- Microsoft Office skill set
- Experience working within health systems preferred.
- Experience with underserved, transient populations
- Unencumbered driver’s license and ability to become an approved driver per agency policy.
- Strong communications skills
GUIDELINES
This position also requires the incumbent to have a valid driver’s license with a good driving record and a private vehicle in order to fulfill the duties of the position.
PHYSICAL DEMANDS
Most of the work can be performed while sitting and talking. The position requires the ability to travel within the community.
WORKING ENVIRONMENT
Working hours are spent indoors in offices or other meeting rooms or in the community Supervision of outdoor activities, visits to community agencies, and participation in staff retreats and staff development activities will be required.
