As the Shared Living Supervisor, you will solicit, hire and train host home providers to share their lives with people who have disabilities. You will assess home settings and support needs to develop a person-centered plan to help people live their best life within the host home. You will ensure compliance with rules and regulations. Here are some of the job duties it will take, to be successful in this role:
- Maintains ongoing communication with participants, staff, guardians, parents, host home families and Conflict-Free Case Managers about ISP plans, policies, concerns, etc.
- Orientates, coordinates and trains Shared Living Providers
- Monitors providers schedules and approves providers invoices
- Monitors completion of mandatory trainings and assessments of shared living providers
- Provides oversight and maintenance of participants records to ensure that data is current and comprehensive
- Monitors participants for safety, including assurance that each home environment is safe, clean and in good repair
- Provides appropriate ongoing feedback, both formal and informal, to host home providers
- Completes Temporary Plan of Care in partnership with the Conflict Free Case Manager when person is transitioning to host home setting
- Assists the gathering and delivery of quarterly monitoring documentation to CFCM and other appropriate parties
- Ensures that the person-centered plan is being implemented as written, and communicates with case manager when the plan needs to be changed or updated
- Reviews the host home documentation to ensure completion and accuracy
- Coordinates medical appointments, including attending important or necessary appointments as needed
- Assists host home families in finding respite care when needed
- Participates in incident reporting management including review of all incident reports and making appropriate and required notifications
- Reinforces and ensures host home families comply with Health Insurance Portability and Accountability Act
- Makes themselves available and responsive to host home families in emergencies and crisis situations
- Assess’ the need to intervene and aids in resolving conflict between host home families, parents/guardians and the individuals
- Conducts face to face visits each month with people living in host homes, and completes required site documentation for each visit
- Facilitates interviewing/contracting for Shared Living Providers
- Participates in evaluation process of vetting all host home families

EDUCATION AND EXPERIENCE
High School/GED degree required. Bachelor’s degree in Human Services, Healthcare, or Business-related field preferred. Four or more years’ experience in related field, or a combination of education and experience providing equivalent knowledge. Valid driver’s license with clean driving record.
