SUMMARY:
Lifeworks, Inc is a distinguished human services agency working with individuals who are intellectually challenged and developmentally delayed in a variety of community settings.
Work as a CASE MANAGER (DIRECT CARE). We are looking to add a few members to our team that will be responsible for our interdisciplinary team caring for people at our Lifeworks Employment Services in West Roxbury, MA.
Case Management responsibilities:
- Writing and implementing Individuals Support Plans (ISP) as well as completing all documentation
- Maintains regular communication with individuals as well as all members of the ISP team.
- Daily completing of payroll and billing documents
- Weekly data tracking of goals and objectives
- Monthly progress notes for individual goals and objectives
- Maintains electronic case files for caseload (Icentrix) as well as modified physical case records (mini binders).
Supervise and train individuals at independent employment sites:
- Train people in the appropriate work skills for the job position
- Act as a liaison between Lifeworks Employment Services and the employer
Provides staffing and job coaching to Group Supported Work Sites:
- Operates company van in a safe manner within the legal guidelines of the State of Massachusetts
- Assists individuals in meeting the expectations of employers at GSWS
Provides staffing for on- and off-site Life Enhancement Curriculum:
- Assists people served in the areas of employment, social, creative, and health and wellness development and training.
- Leads individuals served in community volunteer and learning experiences.

- A copy of a Covid-19 vaccination of two shots will be required
- BA/BS in Human Services / related field or substitute 10 years of related experience with persons with ID/ DD or human services.
- Effective oral and written communication skills
- Car/Valid driver’s license in State of residency.
All CASE MANAGER candidates must be able to successfully pass a CORI, reference, multi-state background, and driving record check.
