- Master's degree or higher
- Nurse Practitioner (4 years)
- 4 years of experience in ANY of the following:
- Geriatric Care
- Geriatric/LTC
- Valid American Heart Association Basic Life Support (AHA BLS)
- Valid Nurse Practitioner (NP) in the state of California (CA)

- Provide direct medical care to PACE participants
- Complete sections of assessments and care planning
- Manage a panel of participants
- Initiate, evaluate, review and change orders for medications
- Assess, initiate and coordinate care on a daily basis under supervision
- Document all participant interactions, assessments, next steps in the EMR within the specified timeframe set by the center
- Make referrals to specialists with approval and request consults as needed
- Perform history and physicals and participant teaching
- Order, interpret and follow up on lab tests and results
- Perform selected procedures
- Recognize and escalate abnormalities as needed
- Liaise with other team members for participant’s care, including behavioral health and pharmacy
- Evaluate and treat participants where they are located, whether that be in the center, home, virtually, SNF etc
- Facilitate advanced planning discussions with participants
- Document chronic conditions via HCC capture Schedule and Shift Details
- 40 hours per week, no on-call. Schedule will vary.
