*How Is This Role Important to Our Work?

The Livio chaplain will be a key member of Livios interdisciplinary teams and will work shoulder-to-shoulder with providers, nurses and social workers. The chaplain will collaborate with these team members to address patients and caregivers spiritual and psychosocial needs. This is a unique opportunity—a chance to innovate, work in collaboration with talented colleagues, and improve quality of life for patients and their loved ones.

A Day in the Life:

• Provide expert, home-based spiritual care to patients with advanced illness, including complete bio-psychospiritual assessment and monitoring of spiritual outcomes.
• Connect patients to faith group resources, taking into consideration patient faith preferences and background.
• Collaborate with providers, social workers and other clinicians to ensure high-quality spiritual care.
• Participate in regularly scheduled interdisciplinary team meetings.
• Participate in care planning meetings with patients, their family members and caregivers.
• Represent Livios mission and clinical teams in partnerships with local and regional faith-based organizations.

• Strength in time management and the ability to handle multiple tasks and priorities.
• Ability to work collaboratively and independently, in settings without direct real time supervision.
