What are the responsibilities and job description for the Case Manager position at LOWELL COMMUNITY HEALTH CENTER?
Provides case management inclusive of social services and medical coordination to clients infected with HIV. Ensures that all clients have access to the highest quality of care and delivers all services in a culturally competent manner.
Maintain Case Load of not less than 30 HIV consumers
Demonstrate regular communication with case load
complete initial assessments as mandated by payer sources
Complete re-assessments as mandated by payer sources
Complete individual service plans in a measurable manner
Complete all required documentation including documentation in the client's medical record, client utilization forms and HDAP forms
Assist with health benefits procurement
Facilitate supported referrals for medical appointments
Facilitate supported referrals for housing, food resources, other concrete social services.
Accompany clients and/or be present for medical appointments
Facilitate supported referrals for behavioral health services
Participate in administrative and clinical supervision as required
Participate in a weekly interdisciplinary treatment team
Co-Facilitate support groups as required
Facilitate/Co-facilitate support activities as required
