Who we are looking for: The ideal candidate will be mission-driven with a desire to make a difference for families and communities served by LCS Northwest.The role: We are looking for a Case Manager to assist refugee, asylee, and immigrant families in becoming self-sufficient through delivery of social services.ESSENTIAL FUNCTIONS:Provides in-home services to refugees including case management, needs assessments, cultural orientation, crisis support, information and referrals as needed. Provides parenting curriculum and educational support, using the Active Parenting curriculum model. Works with clients to create and administer self sufficiency plans.Assists clients with organizing transportation to and from medical appointments.Provides direct services to assist refugees in problem solving, including bilingual interpretation when needed.Provides coordinated care and services with different departments within the agency as well as outside agencies, organizations, and school districts.Ability to provide after hour crisis management support when needed.Maintains thorough case records for all assigned clients, including case management documentation and related tasks.Maintains a positive working relationship with staff, program participants, volunteers, the general public and any collaborating organizations. This includes activities such as: development of referrals, follow-up and outreach efforts. Administers program evaluation tools and satisfaction surveys to program participants according to the timetables for these activities. Attends weekly individual and/or group supervision, staff and/or team meetings as necessary for the position. Completes assigned assessments related to program services. Submits weekly schedules, timesheets alternate weeks from pay week, and monthly records of mileage and expenses incurred in service delivery. Participates in peer review as part of the Quality Assurance Plan. Promotes the agency and its services in a positive manner through active involvement in program-related community and organizational activities as assigned or available. Completes any other tasks assigned by a supervisor or designated to the position in the program design in a timely manner.Maintains confidentiality in compliance with HIPAA laws and agency requirements. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.Minimum Qualifications:Knowledge of and cultural responsiveness to the African population (including but not limited to refugees from the Democratic Republic of the Congo)Must have an understanding of American culture, particularly with a working knowledge of child abuse preventions models, and Child Protective ServicesMust have excellent communication skills with the ability to read, write, and speak English as well as a relevant African language (Swahili, Kinyarwanda,Tigrinya, Kirundi, Lingala etc.) Must be able to demonstrate good teaching abilities and interpersonal communication skillsComputer skills, including but not limited to ability to use email, Google platforms, and create and maintain electronic case notes.Preferred Qualifications:Bachelor’s degree preferredTwo years of related case management work experience preferredCERTIFICATES, LICENSES, REGISTRATIONS:Requires a valid State Driver’s License, auto insurance, and reliable transportation.BENEFITS: We offer a full benefits package with options for medical, vision and dental insurance.
