Who we are looking for: The ideal candidate will be mission-driven with a desire to make a difference for families and communities served by LCS Northwest. The Role: The Case Manager will provide direct services to the Senior Service program participants around the social determinants of health (SDoH) needs. Essential duties will include:ESSENTIAL FUNCTIONS:Convey the purposes and services of the program to the user population and the impact that program or service would have on their health and wellbeing. Provide culturally and linguistically appropriate case management services to qualified program participants. Assess and address other needs relating to the social determinants of health.Coordinating support groups, workshops, respite care services, and follow-ups to connect clients to resources. This also includes individualized instruction and individual meetings, and ensuring appropriate assessment of safety and ability to meet activities of daily living for seniors.Facilitating referrals to state and federal benefits, nutrition programs, family caregiver programs, kinship care, care coordination, Medicaid case management and other services available to seniorsIn collaboration with the Senior Services Program Supervisor, launching the CoRE Committee to meet via Zoom or in-person in the various language cohorts.Work with paraprofessionals, case aides, and community subcontractors to ensure cultural and linguistic access to participants.Ensure appropriate documentation and gather outcome data under the direction of the Program Supervisor. Assisting with language and interpretation needs as necessaryDevelop and maintain relationships with refugee communities, resettlement agencies, mutual assistance associations, and other community based organizations, and participant in the service networks as necessary. Work effectively with community members (staff, clients, doctors, agencies, etc.) to reduce cultural and socio-economic barriers between clients and institutions.Identify gaps in services and report to the Program SupervisorCommunicate information to clients and providers clearly and succinctly, both written and verbal. Conduct successful outreach and follow up with potential clients Requires strict adherence of HIPAA standards in the office workspace to ensure confidentiality.Perform other duties as assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Minimum QualificationsBachelor’s degree or equivalent work experience in a relevant field. Minimum of 2 year working in a case management or social service role. Minimum of 2 year working with seniors and refugee populations. Bilingual in English/Arabic preferred, English/Ukrainian/Russian,, English/Farsi/Dari or other languages as determined by program needs CERTIFICATES, LICENSES, REGISTRATIONS: Frequent travel in King County. A valid Washington State Driver’s License and auto insurance required. The Perks: We offer a full benefits package with options for medical, vision and dental insurance. Vacation includes up to 2 weeks in the first year, with an increase up to 3 weeks in the second year and incrementally up to 5 weeks after 6 years (based on hours worked).  Up to 2 ½ weeks of Sick leave for you or to take care of your immediate family (based on hours worked). 10 Paid Holidays plus 2 Floating Holidays each year.  A 403(b) Retirement plan, with employer matching contributions after meeting eligibility requirements.  Our Employee Assistance Program (EAP) is designed to help and support you.  Mileage and expense reimbursement is also provided.
