SUMMARY
The Guardianship Case Aide provides supportive services to older adult clients under guardianship with Lutheran Family Services Rocky Mountains (LFSRM). The Guardianship Case Aide meets the immediate and ongoing needs of older adult Wards and works with the team to ensure quality of life for those served.
ESSENTIAL DUTIES AND RESPONSIBILITIES
•Treats wards with dignity and respect and engages clients in activities essential to quality of life.
•Monitors clients' well-being through monthly visits to ensure the proper level of care is being provided.
•Attends medical appointments, care conferences and periodic court hearings.
•Drives or arranges for transportation for clients to appointments when appropriate.
•Maintains frequent contact with the team in order to identify client needs and provide support and assistance when needed.
•Maintains written and electronic records in a timely and organized fashion.
•Participates in the fact-gathering process for potential and current wards.
•Assists the team to maintain compliance with all legal and court related requirements related to the guardian's role with each case.
•Becomes and remains familiar with guardianship roles and responsibilities.
•Participates in community outreach activities that advance the work and message of the Guardianship program.
•Performs other duties as assigned.
QUALIFICATIONS

•Knowledgeable in social work, human services, and/or aging; experience with older adults preferred.
•Skills and sensitivity in working with older adults with cognitive impairments from a variety of cultural and ethnic backgrounds with a variety of challenges.
•Skills in person-centered care, strengths-based approaches, and problem solving.
•Ability to communicate effectively in oral and written forms.
•Detail oriented, organized and able to work in a multi-tasked environment.
•Ability to self-start and work well in a team setting.
• Computer skills including Microsoft Word, use of emails and the completion of computer-based forms.
• Possesses a valid Colorado driver's license; has a reliable and consistently available vehicle; and is able to regularly travel throughout the agency's service area by way of interstate highways and primary and secondary roads, including those passing through the mountainous terrain within the state and region.
• Must carry automobile liability insurance at the level of $100,000/300,000/100,000.

EDUCATION and/or EXPERIENCE
Associate's degree or two years of college or career and technical school; or high school diploma with one to two years related experience and/or training in an aging related field; or equivalent combination of education and experience.
