The Program Director is responsible for coordination and management of care coordinators providing case management to recipients of government health plans. Priorities include supervision, orientation, and ongoing training of high-quality staff and ensuring that all the regulatory requirements programs are developed, implemented, coordinated, and monitored to the highest degree possible.
Annual salary of $67,000-$70,000 with a $2000 hiring bonus!

Job Requirements:

• Bachelor’s Degree, or Associate Degree, or Licensed Social Worker.
• Minimum of 3 years of experience in case management operations and supervision in government health plans and manage care health plans; where one year experience is from Home and Community Based Services.
• License to practice as RN or LSW in state of MN required.
• Driver’s license required.
• Bureau of Criminal Affairs (BCA) clearance is required.
• Motor Vehicle Check (MVR) with a satisfactory driving record required.
• Ability to accept/follow through with direction, recognize boundaries, be creative and suggest/implement strategies to enhance the service, patience, self-control and flexibility.
• Ability to react appropriately in an emergency situation.
• Basic computer skills.

The successful candidate must be cleared by a Bureau of Criminal Affairs (BCA) background study and have a drivers’ license; a Motor Vehicle Check (MVR) is required.
