Position Summary:
Responsible for the abstraction, entry, and submission of data on all trauma patients receiving care at McLaren Macomb. Assists with other aspects of the Trauma Program as assigned.
Essential Functions and Responsibilities:
In conjunction with the Clinical Trauma Registrar, identifies all trauma admissions, transfers, and deaths, using clinical identifiers and inclusion / exclusion criteria defined by the American College of Surgeons (ACS) through emergency department logs.
Runs the daily trauma rounding list of patients to ensure accuracy based on the emergency department admission logs.
Collects data on every trauma patient using a medical center specific abstract form or through direct entry into the registry. Works with various transferring hospitals, EMS agencies, coroner’s offices and other entities as needed for data retrieval.
Ensures accurate and timely entry of data into established database.
In conjunction with the Trauma Department personnel, revises the abstract form to remain current with the required data elements specific to:
NTDB
MTQIP
TQIP
State of Michigan
Maintains integrity of registry. Completes a validation tool to ensure accuracy.
Identifies, uploads, and transfers trauma registry data to approved local, regional, state and national database systems, as approved by the TMD and TPM, such as MTQIP, TQIP, NTDS and State of Michigan according to requests and scheduled dates.
Ensures confidentiality and security of patient data stored in registry files.
Codes medical information using AIS-2005 Injury codes to ascertain appropriate ISS, E-codes, ICD-10-CM and ICD-10-PCS coding. Accurately enters all diagnosis, diagnostics, procedures and complication codes.
Monitors effectiveness and efficiency of current software and hardware. Maintains knowledge of most current options for advancements in the system through articles, seminars and networking whenever possible. Offers proposals for changes when changes are recommended.
Interacts with software vendor to maintain software updates, for problem resolution, and technical support as needed. Contacts internal IT department personnel as needed for assistance in conjunction with the software vendor.
Provides primary support to the Clinical Trauma Registrar, PI Specialist, MTQIP Clinical Reviewer, Trauma Medical Director and Trauma Program Manager.
Provides monthly, annual, and ad-hoc reports to the TMD and TPM, and other departments across the trauma continuum of care as needed.
Contributes to problem resolution and positive working relationships among health team members.
Recognizes and follow lines of authority / communication within the work setting and works within those limits. Know when to defer decisions to others.
Demonstrates knowledge of ACS, JCAHO, CMS and other regulatory body requirements with regards to corporate compliance.
Participates in local and statewide Trauma Registrar Committees.
Performs other related job duties as requested.
Qualifications:
Required:
High School Diploma or GED
2 years data abstraction / entry. Experience with registry software preferred. Must possess hospital or medical field experience, including a solid understanding and working knowledge of medical terminology and anatomy and physiology.
Successful completion of a Trauma Registry Course and AIS-2005 Course within the first year of employment
Preferred:
Associates degree in health-care related area
Certified Coding Specialist
Previous Registrar and / or medical record experience
Achieve and maintain trauma registrar certification.
