Position Summary
This position is 5 days/week, weekdays only, typically 8am-4:30pm; this is a hybrid position with some remote work days.
The Health Coach Care Manager is part of an interdisciplinary team providing complex care management to our most vulnerable community members. The team is embedded in the MMP Clinics located in Portland, a dynamic setting where you’ll work with a diverse population of patients as well as medical residents completing their clinical rotations. Ideal candidates have prior experience in an outpatient setting, an interest in community health, and experience working with language interpreters. Multilingual candidates are encouraged to apply. Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: High School Diploma preferred. Bachelor’s Degree preferred.
- License/Certifications: Valid Maine Driver’s License. Health Coach Certification preferred.
- Experience: 2 years' working in a social service or health care setting or equivalent educational setting preferred.
- Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
- Bilingual preferred.
- Sensitivity to cultural and ethnic diversity preferred.
- Local community resident with good knowledge of the resources available in the community preferred.
- Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
- Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
- Familiarity with medical terminology preferred. Ability to project a pleasant and professional image even under stressful situations. Ability to maintain confidentiality.
- Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
- Ability to travel frequently to PCP practices and other sites where patients receive care. Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.
