What are the responsibilities and job description for the Social Worker III, Maine Medical Center Kidney Transplant Program position at MaineHealth?
- Education: Knowledge of social work/clinical counseling theories, techniques, and therapies as used in individual, family, and group treatment, as well as principles of social welfare as acquired through completion of a master's degree in social work/clinical counseling from an institution accredited by the Council on Social Work/Counselor Education.
- License/Certifications: Licensure by the Maine State Board of Social Work Licensure as LCSW (licensed clinical social worker) or LCPC (licensed clinical professional counselor).
- Experience: Four or more years work experience in a health or mental health setting, at least three of which employed by MMC/SHH.
Position Summary
This is an advanced position for the proficient social worker/clinical counselor (SW/CC) with at least 3 years experience at MMC/SHH. Basic duties involve evaluation of psychosocial needs of patients/clients and their families, and provision of diagnostic/preventive and treatment services including individual, family, and group therapy. Also provides complex discharge planning, coordinates aftercare services, and makes referrals for other community services based on the treatment plan. The employee works in collaboration with other health/mental health professionals and provides consultation to the treatment team regarding patient/family social and environmental needs. The SW/CC III role requires advanced clinical skills, and responsibility in 3 of 7 competency domains. The employee in this position has the ability to care for any patient in their proficiency area and to function independently. May supervise other SW/CCs, students, or other staff. Contributes solutions to network problems and program commitments. Is able to orient new staff and handle crises in patient care. Work is performed under general supervision in accordance with institutional and social work/clinical counselor policies.
