SUMMARY
Provides assistance in Resident care planning activities, including the MDS and MMQ processes. Participates in timely completion of MDS process (Minimum Data Set, discharge and re-entry tracking forms).
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
· Assists the Resident Assessment Coordinator in fulfilling responsibilities, as directed.
· Completes assessments of Residents that directs the plan of care.
· Conducts orientation sessions on the MDS process to all employees involved.
· May coordinate information systems operations and education for the clinical department.
· Responsible for contacting Medicare benefits hotline as required per company guidelines.
· Develops schedule of due dates for periodic assessments.
· In the absence of the Resident Assessment Coordinator, may perform duties of that position with oversight by a Registered Nurse (if applicable).
· Completes required forms and documents in accordance with company policy and state and/or federal regulations.
ENTRY QUALIFICATIONS
· Graduate of an accredited school of Nursing.
· Valid State registration as an LPN/LVN or RN.
· Knowledgeable of OBRA, Perspective Payment System, and MDS process.

Employee will occasionally assist to lift and / or transfer patients weighing between 100 to 250 pounds. Employee may occasionally be exposed to blood or other body fluids. HOURLY WAGE RATE RANGE: $40.00 to $45.00 DEPENDS ON EXPERIENCE
