Job description:
With general direction from a Family Case Manager, the Case Manager Assistant assists in planning, organizing, and documenting the medical conditions of prenatal clients both in home and clinic settings. The candidate will manage client correspondence and ensure that all program standards and procedures outlined by the Illinois Department of Public Health, Department of Human Services, and East Side Health District are met.
The Case Manager Assistant will investigate and establish contact with individuals, families, or community agencies to facilitate the delivery of services offered through local healthcare agencies, and social and human services agencies. The Case Manager Assistant will provide ongoing services to clients as needed and work collaboratively with professionals in other disciplines to ensure continuity of health care.
This position requires local travel including outreach activities and occasional out-of-town travel for meetings, conferences, etc.
Qualifications
Required Skills, Education, & Experience:
- One year of experience providing services to pregnant women and families with young children.
- Skilled in maintaining client medical files, both electronic and hard copy.
- Thorough knowledge, skills, and experience working with and utilizing community resources.
- Familiar with local health and human services agencies.
- Computer literacy at the user level is required; experience in scheduling is a plus.
- Ability to maintain satisfactory working relationships with other employees, patients, family members, health personnel, and other professionals.
- Preferred bilingual speaking/writing proficiency (Spanish).
- Must own a dependable automobile properly insured and possess a valid drivers license.
- Willing to accept other duties as assigned by ESHD.
- Adheres to policies and procedures, confidentiality, and HIPPA principles.
- Drug-free environment; drug screening and background check required.
