McLean Affiliates is looking for a 40 hour a week Social Worker to assist with assessing our patients needs - psychosocial, environmental, and financial. Responsible for the resident management process by collabortating daily with the IDT members involved in the care of residents. Ensuring resident rights are maintained at all times especially during care transitions.
Must have MSW or BSW and possess one year of social work experience in a health care setting preferably in long term care/post acute care.
