Join the Medwiz of New Jersey Pharmacy team as the Clinical Pharmacy Director where you will establish drug usage and quality specifications, promote effective drug therapy and performance standards, plan and organize pharmacy policies and procedures and support regulatory compliance.

Duties:
Assumes responsibility for effective pharmacy services within Medwiz Pharmacy’s contracted long term care facilities.
Ensures facilities remain compliant with federal and state regulations
Ensures facilities comply with Medwiz Pharmacy policies and procedures
Conducts medication regimen reviews and evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and positive clinical outcomes. Makes recommendations for therapy changes to physicians and facility staff as indicated
Maintains patient/resident database to include medication history and appropriate clinical data, including psychotropic medication changes and gradual dose reductions
Assists facility with tracking and accountability of controlled substances
Participates in QAPI, medical staff, antibiotic stewardship, psychotropic monitoring, P and T committee and cost containment meetings at each facility as needed.
Composes and Submits written reports where applicable in a timely manner
Participates in other meetings at the facility as needed.
Provides drug information and addresses clinical and regulatory questions of facility staff
Collaborates with the healthcare team to provide clinical support as needed.
Actively participates in facility formulary management and streamlines out of pocket medication expenses
Promotes the clinical pharmacy development of facility staff through clinical leadership and education and competency training.
Supports facilities during state and federal surveys to reduce the potential for medication-related citations
Assists with post survey plans of correction to ensure compliance when necessary
Observes medication administration by nursing staff, monitor for medication errors, perform medication cart and storage area audits and provide written report of findings.
Acts as a liaison between facility nursing staff and pharmacy management to communicate facility needs and ensure pharmacy services are being provided effectively.
Communicates pharmacy clinical updates and policies to facility staff.
Assists with updates to Medwiz Policy and Procedure.
Collaborates with account reps to ensure facility is receiving pharmacy services that meet or exceed expectations.
Attends QA meetings at the pharmacy as determined
Work Location:
Work in the field at various long term care facilities on a rotating schedule
Must maintain adequate provisions for remote work from a home office
Must have unrestricted access to a reliable vehicle for transportation
