Purpose Statement/Position Summary
The Social Worker, MSW (Per Diem) is responsible for providing interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care. The social worker has the responsibility to obtain, plan, and communicate with the team, peers and supervisor utilizing all methods of communication and adhering to policies and procedures.
Essential Job Outcomes & Functions
Manages a specific caseload of patients as identified by the Social Work Service Department. Evaluates and assesses to determine those social, emotional, financial and cultural factors of the patient’s life situation that may contribute to the total diagnosis and treatment plan for the patient. Demonstrates a comprehensive and systematic approach in providing a variety of therapeutic and intervention modalities to remove psychosocial barriers that prevent a safe and timely transition through the continuum of care.
Collaborates with the case manager and the healthcare team to provide education, resources, and referrals to patients, families and caretaker. Assures that psychosocial and socioeconomic needs are addressed in the treatment or discharge plan. Facilitates coordination among health care professionals, services, and settings involved in the patient’s care, with a focus on enhancing patient satisfaction through assuring that the rights of the patient and family are respected and maintained throughout the continuum of care.
Maintains professionalism and provides ongoing education to the healthcare team regarding the social workers role and psychosocial issues as it relates to patient care.
Educates, facilitates and coordinates with the patient, families, caretakers, and community agencies to ensure that resources available to the patient are in place to prevent readmission to the hospital; assuring effective and efficient utilization of private, public, and philanthropic funds.
Responds to and completes psychosocial services in a timely and efficient manner, causing no avoidable delay in the implementation of the treatment plan or in the anticipated discharge time or date. Manages complex bio psychosocial cases to expedite patient/family follow through of treatment plan.

Qualifications/Work Experience
1 years of direct social work experience in an acute healthcare setting preferred. Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist. Each Social Worker, MSW is assessed at orientation and annually using these tools.
Education/Licensure/Certification
Master’s of Social Work (MSW) Degree from an accredited School of Social Work required. California Children’s Services paneling required within 90 days of employment.
