MemoryCare: Dementia Care Manager for Outpatient Program
MemoryCare is a community-based, out-patient nonprofit organization providing medical care for older adults with cognitive disorders and support/education for their family caregivers. Our nationally recognized model utilizes a team approach to provide comprehensive care for patients and families. Teams have included physicians and Care
Managers from multiple disciplines including social work, nursing, and rehabilitation assistant backgrounds.
Candidates with backgrounds in other human services areas with a passion for dementia care would also be considered. Care Managers must be able to work well with others and have strong interpersonal and written/verbal communication skills. Excellent computer skills are essential for managing job duties. Experience with the geriatric population is helpful but enjoyment of this population is essential.
Care Manager:
- Direct Supervisor: team physician
- 4 or 5 days per week (32-40 hours), Mon-Fri 8:30am -5pm with benefits
General Job Duties: The Care Manager is responsible for providing services to patients with dementia as well as support and education for their families/caregivers, telephone triage and support/resource coordination. Excellent computer skills are necessary. The care manager for each team ensures that all necessary medical records are requested prior to initial enrollment of families and that charts are preloaded with available information from all clinical visits. Care Managers provide initial interaction with families and verify completion of all necessary paperwork. Care Managers conduct the initial intake interview that includes medical and psychosocial history and cognitive testing (training provided). Care Managers are responsible for scheduling follow up evaluations and other administrative tasks as necessary. Care Managers assist families with accessing medication assistance, respite services, in-home assistance, and more based on individual patient/family needs. Interim communications with families via phone or email are managed by Care Managers in conjunction with the team physician. As a non-profit organization, all staff assists with a variety of needs upon request.
Specific Care Manager Duties (list may be incomplete):
- Present a pleasant reception to anyone interfacing with our program
- Greet and check-in patient/family upon arrival for appointments
- Enter patient and family information into the data base
- Input of initial historical data into electronic health record/cognitive testing of patient/vital signs/family introduction to resource center/assist physician in physical exam and necessary procedures such as nail care/preparation of handouts/education for families
- Collect fees due at time of visit
- Schedule follow-up visits
- Assist with proof and format of reports- assist with mailing and logging when office assistant is out.
- Care management duties: responsive to all patient/family related queries via phone, email, or electronic medical record messaging and handling triaging, as appropriate, to physician staff; assist families with resource needs; written documentation of care management interactions for physician review and direction. Uploading email and phone messages into the electronic medical record for their clinical team and notifying team MD to review.
- Phone triage and scheduling when scheduler not available
- Provide resource assistance to MD’s for educational purposes (literature/info searches; community resources)
- Organize and maintain patient/family education files
- Participate in community outreach and development activities on as needed basis
- Assist with resource management in the Eldercare Resource Center
- Other duties as assigned
- Salary commensurate with education, training, and experience
