We are currently seeking a Director of MDS/Care Planning to lead the MDS/Care Planning Department at the Marion and Bernard L. Samson Nursing Center, our 180-bed skilled nursing facility.
- The Director of MDS/Care Planning is responsible for the design, implementation, and oversight of systems and processes related to accurate and timely completion of the RAI (MDS 3.0), and for maximizing the role of the RAI in enhancement of quality of care and resident quality of life.
- The Director of MDS/Care Planning is responsible for the overall PDPM process and development of a skillable level of care, proper diagnoses, NTA scores, and the corresponding appropriate plan of care.
- The Director of MDS/Care Planning works closely with Menorah Manor leadership to develop Federal 5-star rating strategy, and in particular collaborates on initiatives toward quality improvement centered around Quality Measures.
- The Director of MDS/Care Planning is responsible for the supervision, training, and development of nurse Care Planners, as well as RAI/MDS3.0 training and development for all staff of the Samson Nursing Center.
- The ideal candidate will have a thorough knowledge of RAI/MDS 3.0 best practices, PDPM system, the ability to seek out new and more challenging benchmarks, and the drive to innovate; the position requires mission commitment and a sense of ownership of all functions of the department, as well as the ability to integrate seamlessly with the interdisciplinary team.
- The ability to communicate frequently and effectively with staff at all levels is critical to success in the position.
- Must have strong customer service skills and ability to work to team build.
Requirements
- RN with an active, unencumbered Florida License
- Skilled nursing facility experience
- Expert knowledge of MDS 3.0/PDPM system
- Prior successful management experience, especially with regional or multi-site responsibility, is preferred
- Strong working knowledge of Point Click Care/EMR system
- Resident Assessment Coordinator certification (RAC-CT) preferred, but not required
