MHS is looking for experienced Case Managers (CPST) to serve the Clark County area in our Outpatient (Behavioral Health Rehabilitation and the ACT (Assertive Community Treatment) Teams, and in Madison County Outpatient SUD and Youth. Case Managers work FT, Monday-Friday helping clients in the Springfield, London and surrounding areas. Hours may vary based on client needs and schedule flexibility. Assertive Community Treatment (ACT) is a form of community-based mental health care for individuals experiencing serious mental illness that interferes with their ability to live in the community, attend appointments with professionals in clinics and hospitals, and manage mental health symptoms. This position is located in Clark County
Outpatient Case Managers (CPST) facilitate the rehabilitation process for clients and families; monitoring and will coordinate the delivery of necessary evaluations, assessments to identify the patient needs, development of the rehabilitation plan with patient and significant others, linkage and coordination of appropriate community services. Assists with independent skills training; advocacy for financial, medical and legal entitlements. Arranges for crisis intervention. These positions are located in both Clark and Madison Counties.
Qualified candidates will have:
- High school diploma or equivalent with 3 years of related experience -OR-
- Associates Degree in a social services field -OR-
- Bachelors Degree (preferred) in a social services field
- Experience with a mental health/behavioral health population or prior case management required
You MUST have a valid Ohio driver’s license with an insurable, clean driving abstract. Case Managers utilize their own vehicles for transportation of clients and must provide proof of insurance at Ohio minimum standards. You receive mileage and an insurance allowance yearly.
In complying with CMS conditions of participation, all MHS employees are required to be fully vaccinated against Covid-19.
MHS provides CPR/First Aid and NVCI (CPI Blue) for all new staff, along with on-going education and on-the-job training opportunities.
All MHS candidates are required to have an Ohio BCI check (FBI check required if you have lived in Ohio for less than 5 years, or for working with children), 5-panel drug screen and 2 step TB (or proof of prior TB) upon conditional offer of employment.
