The team leader establishes, administers, and directs the program of the Assertive Community Treatment Team, a self-contained clinical team that assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified clients with severe and persistent mental health illnesses: supervises and evaluates the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program clients; and functions as a practicing clinician on the team.
General Responsibilities to include but are not limited to:
- Direct the day-to-day clinical operations of the ACT Team including coordinating staff work schedules to ensure proper coverage is met.
- Facilitates daily team meetings and maintains a daily log of meetings
- Will be second on call for the crisis.
- Maintains hospital and incarceration log
- Direct and coordinate the client admission process and treatment, rehabilitation, and support services in coordination with the psychiatrist.
- Scheduling admission interviews, developing and coordinating initial assessments and treatment plans.
- Direct and coordinate assessments of the following:
- Family Structure and relationships
- Substance use assessment
- Education and Employment
- Social development and functioning
- Daily living activities
- Nursing Assessment
- Psychiatric Assessment
- Provide clinical supervision of ACT team members as well as general oversight and supervision of the assessment and treatment plan for each client. This includes weekly individual supervision of team members and at least monthly, this should occur in the field.
- Initiating and maintaining relationships with staff, clients, community resources, and other outside human service agencies as needed.
- Ensure client medical records are in compliance with agency, Medicaid/Medicare, and any outside governing agencies policies and standards as well as ensure that all third-party payment requirements have been met.
- Perform on-call duties as required.
- Assist with staff recruitment, interviewing, hiring, performance evaluations, CQI team, Global Monitoring Process, enforcing company policies, staff training, and other employee relation issues as needed.
- Review of program financials as needed.
- Performs other essential job functions as required.
- Participate in cross-training with other specialists as deemed necessary by the supervisor.
Rate of Pay:$59,400.00

Requirements:- Masters in Social Work, Psychiatric Rehabilitation, or Psychology.
- This candidate also must have strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses.
- 1 year of supervisory and/or management experience is required.
- This candidate must be license-eligible.
